Evaluation & Planning
We review X-rays and examine the tooth. We discuss your replacement options before proceeding.

Anesthesia
Local anesthesia is administered. You will feel pressure during the procedure but should not feel pain.

The tooth is gently loosened and removed. Surgical extractions for impacted or broken teeth require a small incision.

When is extraction necessary?
Extraction is always a last resort. In some cases it is the most appropriate clinical decision:
- Severe decay that has destroyed too much tooth structure to restore
- Advanced gum disease that has caused significant bone loss around the tooth
- A cracked tooth that extends below the gumline and cannot be saved
- An impacted tooth — most commonly wisdom teeth — causing pain, infection, or crowding
- A tooth that has failed after previous treatment and cannot be retreated
Simple vs. surgical extractions
A simple extraction is performed on a tooth visible above the gumline and can be removed with dental instruments under local anesthesia. A surgical extraction is required when a tooth is impacted, broken at the gumline, or has curved roots. A small incision is made in the gum tissue to access the tooth. Both procedures are more comfortable than most patients expect.
Replacing the tooth
Whenever a tooth is removed, we discuss replacement immediately — because the sooner replacement is planned, the better the outcome. Leaving a gap allows neighboring teeth to shift and the jawbone to begin deteriorating at the extraction site.
